- Your company name and USDOT number
- The certificate holder’s name, address, and any required wording (additional insured, waiver of subrogation)
- For changes: the year, make, and VIN of the unit, or the driver’s name, date of birth, and CDL number
Changes aren’t in effect until the insurance company confirms them. We’ll let you know when they are.
